You authorize regularly scheduled debits to your checking account. Your account will be debited the amount indicated on your billing statement. I acknowledge that the origination of ACH transactions to my account must comply with provisions of U.S. law and agree not to dispute this recurring billing with my bank so long as the transactions correspond to the terms indicated in this authorization form.

Opt-out Auto Pay

To opt out of the auto-payment system, please complete the form to submit your request. Upon receiving your written notice, we will acknowledge your request via email within 48 hours, confirming the cancellation of the auto-payment service.

Opting Out of Autopay

To opt out of the auto-payment system, we require a written notice submitted no less than 10 days before the scheduled payment date. This notice should clearly state your intention to discontinue the auto-payment service.
Payment Reminders
A reminder for scheduled auto payments will be sent to you:
- 7 days before the payment date
- The day before the payment date
Missed/Declined Payments
In the event of a missed or declined payment, notifications will be sent:
- The day after the declined/missed payment.
- Three days following the declined/missed payment.
Fees for Declined Transactions

Please be aware that any declined or returned transactions will incur a $36.00 return fee for each failed transaction.
